Patient received daptacel was supposed to receive kinrix but received both vaccines together; This non-serious case was reported by the other health professional via other manufacturer and described the occurrence of overdose in a 4-year-old male patient who received DTPa-IPV (Kinrix) for prophylaxis. Co-suspect products included Diphtheria vaccine toxoid, Pertussis vaccine acellular, Tetanus vaccine toxoid (Daptacel) (batch number 4CA03C2, expiry date 31-OCT-2025) for prophylaxis. Concomitant products included PROQUAD. On 05-DEC-2025, the patient received Kinrix and Daptacel (intramuscular) .5 ml. On 05-DEC-2025, an unknown time after receiving Kinrix and an unknown time after receiving Daptacel, the patient experienced overdose (Verbatim: Patient received daptacel was supposed to receive kinrix but received both vaccines together). The outcome of the overdose was not applicable. Additional Information: GSK Receipt Date: 20-JAN-2026 The reporter stated that no past medical history had been provided. The patient received Daptacel in the left thigh. The patient had been scheduled to receive Kinrix but received both vaccines on the same day. The reporter indicated that the Daptacel dose had been administered on 05-DEC-2025 and that the vaccine had expired on 31-OCT-2025. The batch number was not provided and a request for this information has been made.
