On an unknown date, the patient received Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: she gave the Men B portion and did not mix it with the ACWY portion) and inappropriate dose of vaccine administered (Verbatim: she gave the Men B portion and did not mix it with the ACWY portion). Meaning, she gave the Men B portion and did not mix it with the ACWY portion. This dose was administered and she was curious as well if this counted as a Men B dose or not. The batch number was not provided and a request for this information has been made.
