Dose potentially administered with white visible chunks; Dose potentially administered with white visible chunks; Pharmaceutical product complaint; This non-serious case was reported by a nurse via call center representative and described the occurrence of product appearance cloudy in a patient who received HAB (Twinrix) (batch number 429JJ, expiry date 01-APR-2026) for prophylaxis. This case was associated with a product complaint. On an unknown date, the patient received Twinrix. On an unknown date, an unknown time after receiving Twinrix, the patient experienced product appearance cloudy (Verbatim: Dose potentially administered with white visible chunks), poor quality product administered (Verbatim: Dose potentially administered with white visible chunks) and pharmaceutical product complaint (Verbatim: Pharmaceutical product complaint). The outcome of the product appearance cloudy, poor quality product administered and pharmaceutical product complaint were not applicable. Additional Information: Upon internal review the case was updated on 28-JAN-2026 Registered Nurse explained they noticed an unusual appearance on vaccines in a Twinrix box they had white visible chunks, and they did not know for any of the missing 7 vaccines of the same box that were already administered to patients, if they also contained this unusual appearance.
