Potential second dose missing; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of incomplete course of vaccination in a female patient who did not receive Herpes zoster (Shingles vaccine) for prophylaxis. Previously administered products included Shingrix (first dose was received 4 years ago). The patient did not receive the 2nd dose of Shingles vaccine. The patient experienced incomplete course of vaccination (Verbatim: Potential second dose missing). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 23-JAN-2026 The pharmacist mentioned that a patient got their first dose of shingles vaccine 4 years ago, and it was a 2-dose series. Specific date of administration for first dose not mentioned during call. Patient did not get the second dose yet. Caller did not had patient initials or date of birth, or vaccine information since first dose was not administered at their facility.
