attempted to draw up the dose from a new Adacel vial, but was able to draw up only 0.1 mL/then took a second Adacel vial and drew up the remaining 0.4 mL from the second Adacel vial/dose was then administered, with no reported adverse event; Initial information received on 02-Feb-2026 regarding an unsolicited valid non-serious case received from a nurse. This case involves a 57 years old male patient and when attempted to draw up the dose from a new Diphtheria-2/Tetanus/5 Ac Pertussis Vaccine [Adacel] vial, but was able to draw up only 0.1 ml/then took a second vial and drew up the remaining 0.4 ml and was then administered, with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 02-Feb-2026, the patient received 0.5mL of suspect Diphtheria-2/Tetanus/5 Ac Pertussis Vaccine, Suspension for injection (lot U8683AA, expiry date 31-Oct-2027 and strength not reported) via intramuscular route in unknown administration site for Immunization and attempted to draw up the dose from a new vial, but was able to draw up only 0.1 ml/then took a second vial and drew up the remaining 0.4 ml was then administered, with no reported adverse event (off label use) (latency same day). It was reported "The reporter stated that the label goes all the way around the Adacel vial, so it was not possible to see how much liquid is in the vial before attempting to draw up the dose. Reporter stated that the Dance came in a carton of 10 vial, and the first 4 vials were able to be used with no issues, then these two vials were combined for one dose. That leaves four vials remaining in the box. The practice wass concerned that there may be more vials that were short-filled, among the remaining four vials, so they would like to be credited for 5 Adacel vials: the remaining 4 unused vials, and the vial that contained only 0.1 mL." Action taken was not applicable.
