Patient was seen by me PCP 1/14/26 for symptoms that began on 01/03/2026 or 01/04/2026, including neck pain and aching. On 01/06/2026, she started having burning and tingling sensations. She took Aleve and a hot shower, which provided temporary relief, but the symptoms returned. She reports no redness, itching, rash, or swelling at the injection site. She has not experienced any fever but has had headaches at the back of her head. She reports no weakness in her arms or legs but does experience tingling and aching. She has had intermittent chills. She also reports mild nausea, which resolves with water or soda. She does not believe the nausea is related to Aleve as she does not take it frequently. She has tried painkillers and heat therapy, including hot showers, but these have not alleviated the discomfort. She was advised to continue using Aleve or Tylenol, along with heat therapy, for symptom management. She was also advised to maintain hydration and limit caffeine intake. Patient was seen again 2/3/26 for physical. She reports that her tingling symptoms have resolved, but she continues to experience intermittent aching, although less severe than 4 weeks ago.
