Symptom narrative

5 month old patient given Kinrix vaccine, should have administered Polio and DTaP separate. Reivewed w/ Dr., he advised no concerns w/ given vaccine. He advised to contact parents and advise of error and to reassure nothing to be concerned about. MA contacted parents and relayed Dr. written documentation in 2/3/26 visit regarding the vaccine administered.
