Symptom narrative

Patient given IPV and DTaP in left thigh, one of which the dose was not fully given as evidenced by wet spot on exam table next to patient's leg near injection spot. Pt was squirming and crying during vaccine administration. Upon notice, this RN notified charge nurse as well as patient's provider Dr. [name withheld]. Dr. [name withheld] then spoke with patient's mother; plan to do titers in a few months.
