No documented history of vaccine allergy. Fluzone administered on 1/29/2026@17:03. Per report, pt started to experience slight swelling x 1 finger on left hand and 1 finger on the right hand and itching to both hands last night. On call NP was notified with no new orders received. Nurse noted on 1/30/2026 around 4 AM that patient had swelling to right side of face/upper lip and on-call NP gave orders for Benadryl 25 mg PO which was administered. 1/30 day shift nurse noticed patient has significant swelling noted to entire upper lip and right side of lower lip, now also swelling to bilateral hands and fingers, also slight edema noted in bilateral upper extremities as well. Attending notified @ 0645 AM who ordered IV hydrocortisone 50mg which was then administered @06:58 AM. changed to hydrocortisone 50 mg IV push instead. Med administered without any problems. Patient did not experience any respiratory distress, O2 saturation @ 98% on room air, but no improvement to swelling. Attending was notified again and gave order for 0.5 mg of Epinephrine IM (which was given@07:06 AM) and send patient to acute care hospital via ambulance. Director of pharmacy contacted the patient on 2/4/2026 to follow up. Pt report he was discharged from the acute care hospital after being monitored overnight there. The swelling is gone per patient. Pt reported that acute care hospital physician thinks it may have been caused by either the flu vaccine or the lisinopril patient was taking. However pt states he has taken many flu shots in the past without any reaction, as well as been taking lisinopril for over 10 years without reaction.
