On an unknown date, the patient received Engerix B adult. On an unknown date, an unknown time after receiving Engerix B adult, the patient experienced overdose (Verbatim: Possible overdose (adult dose to adolescent)) and adult product administered to child (Verbatim: Possible overdose (adult dose to adolescent)). The outcome of the overdose and adult product administered to child were not applicable. The pharmacist had obtained the lot number and national drug code number of the vaccine. According to the lot number, medical information had searched under the batch lookup and confirmed it as 10 mcg. However, as per the national drug code number medical information had searched in the prescribing information, which stated it as 20 mcg.
