fifty four year old patient was administered a KINRIX vaccine instead of a Tetanus, Diphtheria, and Pertussis; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of wrong vaccine administered in a 54-year-old female patient who received DTPa-IPV (Kinrix) for prophylaxis. Co-suspect products included DTPa (Reduced antigen) (dTpa vaccine) for prophylaxis. On an unknown date, the patient received Kinrix and did not receive dTpa vaccine. After receiving Kinrix the patient experienced wrong vaccine administered (Verbatim: fifty four year old patient was administered a KINRIX vaccine instead of a Tetanus, Diphtheria, and Pertussis).
