she screwed the needle onto a dose of MMRII incorrectly and as a result the needle fell off of the syringe and was left in the patients leg.; She states that the patient received less than the recommended dose; No one was injured; This spontaneous report was received from a pharmacist and referred to a patient of unknown age and gender. The patient's medical history, past medications, concomitant medications were not reported. On 11-Jan-2026, patient was vaccinated with measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) powder and solvent for solution for injection, 0.5ml, total (valid lot #: Z005856, expiration date reported and confirmed as 11-Mar-2027; route of administration and anatomical location was not provided) as prophylaxis. The suspect vaccine was reconstituted with sterile diluent (BAXTER STERILE DILUENT), solution for injection (valid lot number: 2096680; expiration date reported and confirmed as 26-Dec-2027). On that same day, the pharmacist reported that while performing the injection into the patient, she screwed the needle to the syringe incorrectly (wrong technique in device usage process) and as a result the needle fell off of the syringe and was left in the patients leg. She stated that the patient received less than the recommended dose (accidental underdose). No one was injured (no adverse event). The reporter confirmed multiple times that this was an error on her part and stated this was not a defect in the product/syringe (there was not any difficulty attaching needle). In addition, there was no damage to the syringe, no visible defects or issues.
