A 37-year-old female client presented to the clinic for immunizations and received influenza, tetanus, hepatitis B, polio, and MMR vaccines. Approximately three minutes after vaccine administration, the client reported a metallic or vaccine-like taste in her mouth. Two to three minutes later, she reported dizziness. The client appeared pale. The client was placed on an exam table with legs elevated. She denied difficulty breathing. The client reported numbness of the tongue and a sensation of extreme cold in the throat. BP 128/82, 122/76, 112/78, 132/82 Supervisor was notified, and the emergency bag was obtained. The client was administered 25 mg diphenhydramine (Benadryl). The client was observed in the clinic for approximately 2.5 hours. No respiratory distress was observed during the monitoring period. At discharge, the client reported improvement in symptoms, with continued numbness of the tongue. The client was advised to seek emergency care immediately if symptoms worsened or new symptoms developed. Follow-up by phone was planned for the following day. The client left the clinic accompanied by her in-laws.
