Symptom narrative

Pt had a delayed reaction of swelling, tenderness, redness and itching only below the injection site. Pt stated she experienced common side effect of tenderness for the first couple of days and started worsening on the 3rd day and she just came in today (2/5/26) to report and ask about the adverse reaction. I advised pt to take an otc ibuprofen or tylenol if she experiences pain and ice the area to help with swelling. I also advised to see doctor if it gets any worse.
