Symptom narrative

Pt was administered Tdap on 1/21/26 while in clinic. It was not entered into clinic health record but was entered into immunization registry. Provider order the vaccine, MA did not pull immunization registry report prior to pulling vaccine. TDAP was pulled and administered on 2/5/2026. After pt left the clinic, MA proceeded to enter in immunization registry and realized the vaccine had been administered on 1/21/2026. Provider and pt were made aware. No side effects noted while in clinic. MA has been educated on the importance of documentation in the health record and also pulling immunization registry prior to administration of all vaccines.
