patient received Spikevax (Moderna) 2025?2026 formula at Drug store. A post-administration inventory review identified that this unit had exceeded the 60-day refrigerated storage limit after removal from the freezer (manufacturer guidance: store 2?8?°C for up to 60 days or until printed carton expiry, whichever comes first; do not refreeze). Therefore, the dose is considered expired/invalid. Per CDC COVID-19 vaccine administration errors & deviations guidance, expired/invalid doses should be repeated with an age-appropriate product; the invalid dose does not count toward the schedule. Patient was notified 2/5/26, counseled, and a replacement Spikevax dose was suggested and arranged for. Quality actions: Remaining out-of-date inventory quarantined and discarded per policy. Staff re-educated on beyond-use dating (BUD) expiration of 60 days once removed from the freezer at the [withheld], labeling boxes with this BUD, rotating stock to keep expiration dates used appropriately and checking expiration dates when doing monthly cycle counts.
