Symptom narrative

MMRV vaccine inadvertently given to a patient on 09/19/2025. There was no adverse signs/symptoms reported. Per MD consult, MMRV vaccine may be counted to completion of the MMR and Varicella vaccine series and dose does not need to be repeated. Phone call to patient's parent on 02/03/2026. Parent informed about patient inadvertently receiving MMR vaccine and per MD recommendations patient MMRV vaccine counts towards MMR and Varicella vaccine and dose does not have to be repeated. Parent verbalized understanding.
