Before retrieving the Tdap and HPV vaccines from the refrigerator, I asked the patient if he had ever experienced nausea, dizziness, or fainting after a vaccine. The patient denied prior fainting but reported feeling nauseous and dizzy after a blood draw as a teenager. I asked if he wanted to lay down; the patient declined and stated that it only happened once. Before administering the vaccines, I advised patient that if he does start to feel lightheaded or nauseas to let me know. I administered the Tdap vaccine in the left deltoid. After administering the Tdap vaccine patient stated that he felt fine. I then moved over to the right arm to Administer the HPV. I then advised patient a second time that if he does start to feel lightheaded or nauseas to let me know. I administered the HPV vaccine in the right delt and then moved over to the computer in the room to document that both vaccines were administered after patient confirmed that he was not feeling lightheaded. I advised the patient a third time to notify me if he began feeling lightheaded or nauseous. I remained in the patient room documenting the vaccine administration at the computer. Approximately one minute later, the patient reported feeling lightheaded. While seated, the patient became increasingly lightheaded and exhibited noticeable twitching movements, followed by a brief loss of consciousness. The patient was not moved during this time. I immediately called for assistance. Dr. responded promptly and entered the room. Upon arrival, Dr. assisted in positioning the patient flat on the exam table and evaluated the patient. By the time Dr. arrived; the patient had regained consciousness. The patient was monitored and given apple juice. He remained lying down until symptoms improved. After approximately five minutes, the patient stated he was no longer feeling dizzy. With assistance, he sat up on the exam table and then stood with support. The patient denied dizziness after standing for two minutes. The provider had already given verbal clearance for discharge. The patient was escorted to the lobby.
