Symptom narrative

Patient called in to report work related injury today 2/5/2026. Patient reports receiving a flu shot at Occupational Health Services on 10/30/2025. Patient reports immunization was given "too high up" on her shoulder, reports immediate onset of pain and soreness. Patient reports frozen shoulder (L), reports pain in shoulder has been present and intermittent with L arm use since receiving the vaccination. Patient reports L arm/L shoulder is weaker in comparison to her R arm/R shoulder.
