Symptom narrative

Patient came in today for breast lump and flu vaccine. Pt was given flu form and completed it at check in. Vitals and intake was taken from pt. Reviewed flu form with pt and verbal order given from Provider. Flu vaccine was given with no complications. Pt needed a copy for her work place. Vaccine was documented and then noticed that pt had already received her flu vaccine on 1/2/26. Asked pt and she stated she didn't remember receiving a flu vaccine. Provider and pt notified that she had received 2 flu vaccines this year. Pt advised pt she shouldn't have any side effect and should be covered. Manager notified
