Product preparation issue

Symptom narrative

Medical assistant who was preparing and administering the vaccine did not reconstitute the vaccine in the diluent. Patient was vaccinated with diluent only. This took place on 01/25/26. When the situation was made aware on , the patient's parents and ordering provider were notified today 02/05/26. Patient is not reporting any symptoms at this time. Parent is going to schedule patient to get revaccinated soon, per parent they want to wait a few days before making the appointment.
