Symptom narrative

vaccines administered approximately 2:40- 2:45 pm. Within 1 minute after getting the 3 vaccines patient had a clear runny nose and a constant cough sounding like bronchospasms..Dr notified immediately, and APRN immediately in room. Lungs with wheezing. HR: 180-200, O2 sats 98-100 initially and decreased to 92 %. Epi given at 3:06 pm as per order. Within 30 seconds patient's breathing started returning to normal, and HR decreased to 150's, O2 sats 92-95. Within 2 min of administration O2 sats improved to 96 and above. Oxygen 5 L applied per mask entire time. Patient alert, crying and responsive to stimuli. Patient transferred to ED per EMS. APRN called ED and gave report.
