Symptom narrative

Questionnaire completed by parent. All applicable questions for client answered "No," including Item 1 "Are you feeling sick today?" Parent reported no allergies or negative reactions to previous vaccinations. PHN prepared, documented, and administered (around 16:15) the vaccinations needed for client: 4 total. Patient tolerated vaccinations well, reporting slight pain to the sites but no respiratory distress or integumentary changes noted. After the waiting period, client and parent were brought to the checkout desk, where client threw up (around 16:35PM). Mother reported client told her she suddenly felt ?like throwing up? and did. PHN was informed of episode and guided pt back to an exam room. Patient?s vitals were checked: 16:39 - Temp 98.1 degrees F, pulse 104, 16:45 - B/P 78/54 (Right Arm). During the post-emesis period, client reported feeling better initially, but did become nauseous again. PHN also further asked parent about child's health. She reported pt had an episode of diarrhea that morning, which is not usual for pt. Informed client that this displays further indication to take acetaminophen, as the vaccines given today may give her a fever, body aches, and further nausea if she was already ill. Also used the opportunity of the extended observation period to educate parent about pediatrician clinics in the area and emergency signs of anaphylaxis. After 20 minutes of further observation, the client did not vomit again, nor show signs of respiratory distress.

Current illness

Only upon further assessment did parent mention client had diarrhea the morning prior to vaccinations.
