On 11/29/2024, pt presented to emergency department with left arm pain, and tingling and numbness of the left arm and hand. She stated this began the day following vaccination. She was prescribed gabapentin and prednisone, ad instructed to take acetaminophen and ibuprofen for pain. Encouraged to follow up with PCP and given referral for physical therapy. It is understood that the pt did not follow up with provider recommendations. It is unclear if pt continues to experience paresthesia or if her main concern in contacting public health in Feb 2026 is for assistance with an outstanding bill (her description of her physical issues is inconsistent with an unclear timeline). She did not follow up with public health in the last year, so we were not aware that she may still be symptomatic.
On 11/29/2024, Emergency room provider reports patient had maintained left upper extremity strength. LUE: +AIN, PIN, Ulnar, motor function. No acute deformity noted. Grip strength is equal. Maintained range of motion. Mild tenderness with palpation over deltoid and bicep musculature. No erythema or ecchymosis. X-ray findings on 11/29/2024: no soft tissue swelling. No glenohumeral dislocation. No abnormal acromioclavicular joint space widening. No aggressive bone lesion. No significant arthrosis.
