Symptom narrative

Child's parents were notified about the incorrect immunization the same day immunization was given. Questions were asked about the child if there were any adverse events. The parent told this nurse that the child wasn't having adverse effects. This nurse told the parent to watch the child closely, and take to ER hospital if the child's physical being was not normal for parents and child.
