The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 16-Jun-2025, vaccine of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) was improperly stored at Temperature: 77.7 Fahrenheit (F) for 0 Hours 5 Minutes 0 Seconds. No previous temperature excursion. On 17-Jun-2025, the patient was vaccinated with above Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (0.50 mL/dose, Lot Number: Y019464, Expiration date expiration date reported as 10-Nov-2026, but upon internal validation established as 24-May-2026; strength and route were not reported) for prophylaxis (Product storage error).
