Boostrix-inappropriate vaccine administered to 4.5 yr old patient; a third dose of DTaP but was inadvertently administered a dose of Boostrix; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate age at vaccine administration in a 4-year-old male patient who received DTPa (Reduced antigen) (Boostrix) (batch number 95P4M, expiry date 24-OCT-2027) for prophylaxis. Co-suspect products included DTPa (DTaP vaccine) for prophylaxis. Previously administered products included DTap vaccine (received 2 previous doses of vaccine). On 30-JAN-2026, the patient received Boostrix. The patient did not receive the 3rd dose of DTaP vaccine. On 30-JAN-2026, an unknown time after receiving Boostrix, the patient experienced inappropriate age at vaccine administration (Verbatim: Boostrix-inappropriate vaccine administered to 4.5 yr old patient) and wrong vaccine administered (Verbatim: a third dose of DTaP but was inadvertently administered a dose of Boostrix). The outcome of the inappropriate age at vaccine administration and wrong vaccine administered were not applicable. Additional Information: GSK receipt date: 30-JAN-2026 The reporter reported that Boostrix was inadvertently administered to a 4.5-year-old patient. The patient was in a catch-up DTaP series in which patient had received 2 previous doses in order to start preschool. The patient should have received a third dose of DTaP but was inadvertently administered a dose of Boostrix. The immunization coordinator for the health department had already reported this adverse event to the VARS system.
