Symptom narrative

Expired dose administered; extra fourth dose given; extra third dose given; Late second dose; This non-serious case was reported by a nurse via call center representative and described the occurrence of expired vaccine used in a 66-year-old male patient who received Herpes zoster (Shingrix) (batch number 3959S, expiry date 12-DEC-2025) for prophylaxis. Co-suspect products included Herpes zoster (Shingrix) (batch number EK225, expiry date 10-MAR-2027) for prophylaxis and Herpes zoster (Shingrix) (batch number 274YD, expiry date 15-SEP-2025) for prophylaxis. Previously administered products included Shingrix (received 1st dose of vaccine on 11-OCT-2023 with batch number 274YD and expiry date 15-SEP-2025). On 03-FEB-2026, the patient received the 4th dose of Shingrix. On 23-OCT-2025, the patient received the 3rd dose of Shingrix. On 21-AUG-2024, the patient received the 2nd dose of Shingrix. On 21-AUG-2024, not applicable after receiving Shingrix and Shingrix and an unknown time after receiving Shingrix, the patient experienced drug dose administration interval too long (Verbatim: Late second dose). On 23-OCT-2025, the patient experienced extra dose administered (Verbatim: extra third dose given). On 03-FEB-2026, the patient experienced expired vaccine used (Verbatim: Expired dose administered) and extra dose administered (Verbatim: extra fourth dose given). The outcome of the expired vaccine used, extra dose administered, extra dose administered and drug dose administration interval too long were not applicable. Additional Information: GSK Receipt Date: 03-FEB-2026 Caller mentioned that repeated series for patient 1 was given by accident, since they were not aware that patient already completed a prior series.
