The patient was seen in a homeless shelter, where we have been vaccinating residents due to a meningitis outbreak. The patient notified our clinic personnel that he needed a meningitis vaccine and had not recently had one. He was certain he had received the flu and COVID vaccine, but not the meningitis vaccine. His account had not initially been found in a search of our vaccination tracker but soon was found to have already received the meningitis vaccine from a care van, in the community. So the patient received this vaccine two times within a 10 day period. Clinic personnel were able to find the patient in the shelter and notify him of the event. A vaccination card was provided, so that the patient carries it with him, to hopefully avoid any future error. The patient had no adverse reaction or any symptoms of any kind.
