Symptom narrative

1. Generalized (whole body) pruritus. Taken Cetirizine with some relief however once the medication wears off pruritus returns. Still experiencing on 02/07. 2. Localized pain, tenderness, soreness at injection site radiating to left neck, still experiencing on 02/07. 3. Incorrect vaccine was given at the pharmacy. I asked the pharmacy technician if they had the non mRNA COVID-19 vaccination and they said yes. Then I clearly stated that I wanted the non mRNA COVID-19 vaccine on the consent form. Pharmacy technician during time out asked for my name,DOB, and what I was getting which I said non mRNA COVID-19 vaccine which she then confirmed and proceeded to give me Comirnaty vaccine. I didn't realize the mistake until I updated my vaccination records.
