Reported Symptoms: 10080422:DEUPLICATE THERAPY ERROR; Narrative: Patient presented for follow up primary care appt, told provider that he wanted a flu shot. Provider instructed the nurse at the end of the appointment that the patient requested the flu shot so she retrieved one and administered to the patient. When trying to document she realized patient had already received a flu vaccine the previous October and that what he actually was wanting was a pneumonia vaccine. No noted issues from additional vaccine. Patient safety made aware via error reporting process. Other Relevant HX: DUPLICATE INJECTION
