Symptom narrative

2 IM injections (Pentacel and Prevnar 20) administered Left thigh on 4 month Well Child visit. Parent reported adverse effects at subsequent Well Child check on 2/5/26 stating that she did not want her child to receive any more vaccines due to the adverse effects baby experienced at the last visit from the two injections on one leg: per parent "baby refused to be touched on the leg for the whole day into the night" Parent reported the baby being irritable and the injection site felt "lumpy."
