Purtscher-like retinopathy/ Acute Unilateral Vision Loss; Acute Unilateral Vision Loss; This serious case was reported in a literature article and described the occurrence of purtscher retinopathy in a 58-year-old male patient who received Herpes zoster (Shingrix) for prophylaxis. Literature Reference. Additional patient notes included Patient with no past medical or ocular history. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced purtscher retinopathy (Verbatim: Purtscher-like retinopathy/ Acute Unilateral Vision Loss) (serious criteria GSK medically significant) and loss of vision (Verbatim: Acute Unilateral Vision Loss) (serious criteria GSK medically significant). The outcome of the purtscher retinopathy and loss of vision were resolved. The reporter considered the purtscher retinopathy and loss of vision to be related to Shingrix. The company considered the purtscher retinopathy and loss of vision to be unrelated to Shingrix. Additional Information: GSK received date: 02-FEB-2026 A patient with no past medical or ocular history received the Shingrix vaccine and subsequently developed acute vision loss in his left eye. Examination revealed a visual acuity of 20/150 OS with peripapillary and macular cotton wool spots in a circumferential distribution. Macular optical coherence tomography demonstrated multifocal areas of inner retinal thickening and hyperreflectivity. A systemic laboratory workup was unremarkable. At the 3-month follow-up, fundus findings resolved, and visual acuity returned to baseline. A presumptive diagnosis of Purtscher-like retinopathy associated with Shingrix vaccination was made. Purtscher-like retinopathy may be related to the upregulation of the complement cascade and proinflammatory state after vaccination.
