Symptom narrative

When completing vaccine count, nurse noticed the count was off and investigated. Staff, newer to this process, accidentally provided an adult pt with dTap instead of Tdap. Staff states their verification step was omitted due to busy day/no availability. Staff and team were notified of this error. No concern from pt/family. dTap/Infanrix, 0.5mL Recommendations: Staff to be sure he/she is following the policy with vaccine administration which includes utilization of the vaccine bar code scanner to verify the correct vaccine against the order.
