948 - Mobile vaccine clinic, WC travel to county schools to administer vaccines to students who were deficients in their scheduled vaccine or needed for the up coming school yr. On 2-10-26 clinic was held at school. Student rc'd. Tdap, MCV to the left deltoid and then HPV to right deltoid. 2 secs after administration of HPV child was still sitting in chair. I noticed child became rigid of lower limbs and jerking for 5 10 secs. At this time I held the child to keep from falling out of chair. Once able to move to floor laid her flat was very clammy bp taken 92/48 no S & Sx of any respiratory disstress notied. Dr. notified also let school nurse she notified parents (nurse). Child kept on floor until stable and able to stand. School nurse took child in wheelchair parent on way to pick-up child. Instructed nurse for parent to take child to hospital once. She was picked up. Nurse stated parent will take child home and monitor and then will take to hosp. if needed. 219pm placed call to parent no answer. Left message with my office and cell number for a return call.
