Caller reported light headedness; Caller reported more pain; "feeling terrible"; Information has been received from the patient and refers to herself, a female patient of unknown age. The patient's medical history, concurrent conditions and concomitant therapies were not reported. Historical drugs included first dose of Hepatitis A Vaccine, Inactivated (VAQTA). On an unspecified date, the patient was vaccinated with the second dose of Hepatitis A Vaccine, Inactivated (VAQTA), lot #Z012608 (which has been verified to be a valid lot number for Hepatitis A Vaccine, Inactivated), with an expiration date not reported but upon internal validation establish as 04-MAY-2027, 1 dosage form administered for prophylaxis (strength, exact dosage, route and anatomical site of administration were not reported).
