Symptom narrative

The caller reported no additional AE; A medical assistant called and reported that an adult dose of VAQTA was inadvertently administered to a 22-month-old patient instead of the pediatric dose.; This spontaneous report was received from a medical assistant and refers to a 22-month-old patient of unknown gender. The patient's medical history, concurrent conditions, past medications and concomitant medications were not reported. On an unknown date, patient was inadvertently vaccinated with an adult dose of Hepatitis A Vaccine, Inactivated (VAQTA) suspension for injection, 1 ml, first dose, lot number Z27PB is an invalid lot number, expiration date 27-Aug-2026, for prophylaxis (route of administration and anatomical location were not provided), instead of the pediatric dose (Product administered to patient of inappropriate age). No additional adverse event was reported.
