Symptom narrative

pt states their left arm where injection was given has been red, itchy, warm to the touch and swollen at site of injection. pt experiences nausea, fatigue, chills. sx started tuesday 2/3/2026. pt denies swelling anywhere else in the body, besides localized injection site. pt denies, chest px, sob. provider and nurse informed. all of pt's sx are what is stated in CDC possible reactions. pt agree's given another VIS.

Diabetes mellitus type 2, HTN, generalized anxiety and depression, allergic rhinits, h/o alcohol abuse
