The Arexvy diluent component was only administered to a female patient yesterday-not the full dose/ Inappropriate reconstitution technique; diluent component was only administered not the full dose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate preparation of medication in a 88-year-old female patient who received RSVPreF3 adjuvanted (Arexvy) (batch number ZL4R5, expiry date 16-JAN-2027) for prophylaxis. Concomitant products included Varicella zoster vaccine rgE (CHO) (Shingrix). On 01-FEB-2026, the patient received Arexvy. On 01-FEB-2026, an unknown time after receiving Arexvy, the patient experienced inappropriate preparation of medication (Verbatim: The Arexvy diluent component was only administered to a female patient yesterday-not the full dose/ Inappropriate reconstitution technique) and inappropriate dose of vaccine administered (Verbatim: diluent component was only administered not the full dose). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 02-FEB-2026 The registered pharmacist had called to ask about revaccination or advice on Arexvy vaccination. The patient had also been given a full dose of Shingrix (fully reconstituted) on the same day. The vaccine administration facility was the same as the primary reporter. The lot number provided from the registered pharmacist's end corresponded to the diluent portion of the Arexvy vaccine.
