Symptom narrative

Patient was inadvertently administered the 12+ COVID vaccine (Pfizer brand) instead of Moderna SpikeVax (0.25 mL) at approximately 3:45 PM. No adverse effects were noted at time of administration. Provider was consulted to come back into room and look at patient. No concerns noted. Poision Control was contacted at 4:02 PM- was told to monitor (may have headache, sore leg, fever). Let family know this information.
