Symptom narrative

Approx 30 min following vaccination (10:27) pt complained of "mild trouble breathing" and a headache. Pt was allowed private space to lay down. No improvement after 10 min (10:47). Pt clarified mild trouble breathing was an itchy/scratchy throat. School nurse administered acetaminophen (unknown dose). No improvement after 10 min. (10:57). Diphenhydramine 25 mg was offered, and patient accepted. Pt began feeling a little better after 15 min. (11:15). Approx 15 min later pt was taken home by mother (11:30)
