Symptom narrative

Patient cam into clinic and was due to receive a pentacel vaccine but the medical assistant only gave the liquid portion of vaccine, it did not get mixed with the powder portion. Parent will be called by Dr., notified and asked to return to receive Hib vaccine as instructed by Sanofi spoke to someone -Advised Dtap/IPV counts as valid dose, Hib needs to be given, okay to give asap since this is not a live virus. No symptoms reported and no treatment necessary.
