Symptom narrative

Mother returned to office approximately 40 minutes after MMRV and DTaP/IPV vaccinations. Patient has developed redness of his face and urticarial rash all over his body. No shortness of breath. Lungs are clear, no difficulty swallowing. Prednisolone 15mg po and Benadryl 6.25mg po given in office. Patient monitored in office and noted to be talkative and in no distress at time of discharge. Recommend continued Benadryl every 6 hrs if needed.
