Symptom narrative

Pt stated they started developing hives localized to his face, arms, legs, and back 2 days after getting the vaccine. He took 1 tablet of benadryl which did not resolve symptoms. Hives went away ~3 days after they started. Denies any other symptoms.

Hyperlipidemia, hypertension, and epilepsy

Atorvastatin 40 mg (1 PO AM); Lisinopril 20 mg (1 PO AM); Topiramate 50 mg (3 PO AM & 2 PO PM); Vitamin C (pt unable to recall strength)
