Symptom narrative

Patient received vaccine at a local pharmacy (location unknown). Patient reported that she was hospitalized for 104 fever, "plummeting blood pressure", elevated heart rate, and uncontrolled vomiting.

Medical history

Allergic rhinitis, colon polyps, thyroid nodule, medial meniscus tear, uterine prolapse, history of ductal carcinoma in situ of breast, fibrocystic breast disease, iron deficiency anemia, bilateral mastectomy, restless leg syndrome.

Other medications

calcium carbonate 500mg TID, chlorhexidine 0.12% swish and spit before brushing teeth, pramipexole 0.5mg 1-2 tablets at bedtime.
