Symptom narrative

Patient seen in clinic on 02/02/2026, Kinrix administered to complete vaccines to enroll in Kindergarten. Vaccines later reviewed by another nurse and it was noted that Kinrix was administered outside the 4-6 yr. age recommendation. Nurse called CDC and local state immunization dept. for further recommendations. No adverse reaction noted.
