forgot to mix the diluent in the syringe with the powder; forgot to mix the diluent in the syringe with the powder; used Merk Sterile diluent and mixed it with the GSK vial powder; This non-serious case was reported by a consumer via call center representative and described the occurrence of inappropriate preparation of medication in a adult female patient who received MMR (Priorix) (batch number 58260082415, expiry date 01-MAR-2027) for prophylaxis. Co-suspect products included MMR (Priorix) for prophylaxis. On 03-FEB-2026, the patient received Priorix and Priorix. On 03-FEB-2026, an unknown time after receiving Priorix and Priorix, the patient experienced inappropriate preparation of medication (Verbatim: forgot to mix the diluent in the syringe with the powder), inappropriate dose of vaccine administered (Verbatim: forgot to mix the diluent in the syringe with the powder) and wrong solution used in drug reconstitution (Verbatim: used Merk Sterile diluent and mixed it with the GSK vial powder). The outcome of the inappropriate preparation of medication, inappropriate dose of vaccine administered, and wrong solution used in drug reconstitution were not applicable. Upon realizing her mistake, she used Merk Sterile diluent and mixed it with the GSK vial powder and administered. They wanted to know this is acceptable or does this make the shot invalid.
