A 51-year-old female patient received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), on 03Feb2026 at 11:30 as dose 1, single (Batch/Lot number: unknown) at the age of 51 years intramuscular, in left deltoid for immunisation. The patient's relevant medical history included: "Blood pressure" (unspecified if ongoing); "anxiety" (unspecified if ongoing). The patient's concomitant medications were not reported. The following information was reported: VACCINATION SITE MASS (non-serious), VACCINATION SITE ERYTHEMA (non-serious), VACCINATION SITE WARMTH (non-serious), VACCINATION SITE SWELLING (non-serious) all with onset 03Feb2026, outcome "not recovered" and all described as "eyebal shaped with raised, swollen and warm/Red, warm, raised lump". Therapeutic measures were taken as a result of vaccination site mass, vaccination site erythema, vaccination site warmth, vaccination site swelling. Additional information: Patient received the last Prevenar 20 vaccines at doctor's office yesterday morning, probably around 11:30 AM. It hurt like shots usually do, was IM injection. felt a little bump under her shirt. She guess around 11:30 am, a huge Welsh was what it was, a big welt that kind of looks like an eyebal shaped with raised, swollen and warm. It was warm, pretty big, probably 2 and a half inches wide by about the length and the width was a good inch. Went home last night, it hurt a little more, did not think much of it. Got up this morning, was lump there. Expected a little bump, just looked red, raised, warm spot, where injection was, probably about 2 inches wide by about 1 inch, 2 inches by 1 inch. It was a small lump last night, just got shot through shirt, did not actually look at it. Figured, was just a little reaction from injection itself. About a half hour ago, lifted up sleeve, to peek at, noticed large, red, guesses more like a welt. Showed coworker, was large, red, in left deltoid. Outcome of event: Would say, at this point, has gotten worse. As far as pain, does not know. Outlined it now, since she knows there is big red spot, to see if it gets bigger. Treatment: Did take 800mg Ibuprofen on way in this morning, around 6AM. Indication: Went to doctor, for annual physical. Talked about vaccine, was recommended, for her age, to received pneumonia vaccine, for people over 50. Never had this before, was first time she has gotten it. Concomitant Products: Took daily medications, does not think they would interfere with it. Patient did not receive any other vaccines on the same date as the Pfizer vaccine or within 4 weeks prior to the first administration of the suspect vaccine.Patient medical history reported as Blood pressure, has had greater than 20 years, since same time as anxiety, has been a long time, has had, probably about 10 years, maybe 15. The information on the batch/lot number for pneumococcal 20-val conj vac (dipht CRM197 protein) will be requested and submitted if and when received.
