Symptom narrative

Donor states small patch of red raised bumps with small dots of blood noted to shirt of sleeve one week after administration of TDAP immunization. Upon assessment today in center, red 1/2 dollar size raised bumps 1/2 inch below injection site noted to left deltoid. Skin is dry and intact with no infection noted.
