On an unknown date, the vaccine Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) experience a temperature excursion (TE) for 48 minutes at -11.75C. On 04-Feb-2026, the patient was vaccinated with the vaccine involved in the TE: Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX), 0.5 mL (lot #Z008428 has been verified to be valid, expiration date reported and confirmed as 06-May-2027) or with (lot #Z013023 has been verified to be valid, expiration date reported and confirmed as 12-Aug-2027) as prophylaxis; reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (dose, route, indication, expiration date, and lot # were not reported).
